The Indian River County Community Health Improvement Plan (CHIP) uses data to identify priority issues, establish goals and objectives and collectively execute action plans to improve health. The plan is the product of continuous improvement through diverse partner engagement.

DOH-Indian River, working with community partners, developed a Health Equity Plan to address inequalities in education and health care access and quality, economic stability and issues with neighborhoods and built environments, commonly known as the social determinants of health.
